How does the Rehabilitation Network work?

The Network is a collaborative partnership between the region’s specialist rehabilitation inpatient units, outpatients and community services.

The focus of rehabilitation is to enable people to live as independently as possible by maximising their clinical outcomes. As the Network provides different levels of specialist rehabilitation care, the patient’s pathway can be tailored specifically for their individual needs. Rehabilitation is different for every person depending on individual injuries or illness and what stage they are at in recovery.

Patients are individually assessed and a rehabilitation programme is developed specific to their rehabilitation needs.

Teams of specialist rehabilitation health and social care professionals will work closely with each patient to reduce the impact of injury or illness and help them to adjust to their condition.

Patients, relatives and carers are encouraged to play an active role within rehabilitation programmes.
